My Health Cards

Talking about our health can be hard sometimes.

The My Health Cards help people with intellectual disability have conversations about their health.

The My Health Cards help health professionals to communicate with adults with intellectual disability in a more inclusive way.

The cards can also help family members and support workers to have health conversations with the person with intellectual disability.

Each card asks a question that will support you to

  • Talk with health workers
  • Get ready for health appointments.
